While many plans cover acupuncture treatment, most are restricted to specific conditions, like pain, including back pain, knee pain, headaches, nausea, etc. Wellness and prevention visits are generally not covered. If your health insurance plan includes acupuncture benefits, we will bill your insurance company for acupuncture services. We can also provide superbills for each appointment if we are out of network with your plan. If you depend on your insurance to pay for your holistic health care, we highly recommend that you use the information below to pre-verify your acupuncture benefits.

How to verify your acupuncture benefits with your insurance plan

Artemis Clinic is in network with a few insurance carriers. Though we may appear to be in network with your specific plan, it does not automatically guarantee that your plan includes acupuncture benefits. To help you determine whether your acupuncture treatment will be covered by your insurance plan, we are providing you with a list of questions that we recommend using when calling your plan’s member benefits department.

A note about deductibles: Some plans require a deductible be met before your insurance will help cover the cost of a service. If your deductible has not been met, it’s important to know how much is left. If you have met your deductible, there typically is a coinsurance that they will provide as a percentage. For example, if your deductible has been met and you have a 10% coinsurance, you would be responsible for the cost of 10% of each treatment fee.

Questions to ask your insurance plan’s member benefit representative

  • Does my plan cover acupuncture treatment?
  • Is Artemis Clinic or Katya Kudryavtseva, LAc in network?  You can provide the following national provider identifier for the provider: 1750837092
  • Is medical referral required?
  • How many visits does the plan cover per year?
  • When does the plan year reset?
  • Is there a copay or a coinsurance?
  • Is there a deductible that applies?
  • If there is a deductible, how much of it has been met?
  • Are there diagnoses that are not covered?

NOTE: The information you will receive is a quote and does not 100% guarantee payment for your acupuncture treatments by your insurance plan. Ultimate determination about payment of services is made by your plan, and in case of non-payment, you will be the one responsible for paying for your cost of treatment.
